Are you aged 75 or over?

From April 2014, all patients aged 75 and over will have a named GP who will have overall responsibility for the care and support that our Surgery provides to them.

PLEASE CONTACT THE PRACTICE TO FIND OUT WHO YOUR NAMED GP IS

This does not prevent you from seeing any other GP in the Surgery and your current care will remain the same.  THANK YOU

When the Surgery is closed, please call 111 for Out of Hours Care & Advice

Please leave a Review for Connaught Surgery on the NHS Choices website.

Thank you
